Job Summary We are seeking a compassionate and motivated Rehabilitation Practitioner to join our expanding community neurological rehabilitation team. The successful candidate will assist our Physiotherapists in the assessment and rehabilitation of individuals with a range of neurological conditions, including stroke, MS, Parkinson’s disease, and acquired brain injuries. This role offers a unique opportunity to work across community and home settings, providing tailored rehabilitation programmes under the supervision of qualified Physiotherapists.
Key Responsibilities
• Deliver and support the delivery of individualised rehabilitation programmes prescribed by a qualified Neurological Physiotherapist.
• Work with clients in their homes or community settings to promote independence, mobility, and functional recovery.
• Support clients with mobility, strength, coordination, balance, and functional training activities in home and community settings.
• Monitor and report on client progress to the supervising physiotherapist, updating records accurately and timely.
• Promote client motivation and adherence to rehabilitation goals through encouragement and clear communication.
• Assist with setting up and maintaining equipment used in therapy sessions.
• Assist in conducting baseline assessments and monitoring clients’ progress using standardised outcome measures.
• Provide basic education to clients and families on safe mobility and exercise techniques.
• Encourage clients to engage in self-management strategies and participate in their rehabilitation process.
• Maintain accurate and timely documentation of interventions and client progress in line with company policies.
• Liaise effectively with team members, clients, carers, and external healthcare professionals.
• Contribute to the development of client resources, activity plans, and group sessions where appropriate.

Rehabilitation Practitioner Person Specification

Essential Requirements
• A degree in Sport Therapy, Sport Rehabilitation, or Sport and Exercise Science (or equivalent).
• 1–2 years of experience working in a healthcare or rehabilitation setting – ideally with individuals with long-term or neurological conditions.
• Understanding of rehabilitation principles and the role of physical activity in long-term condition management.
• A genuine interest in neurological rehabilitation and person-centred care.
• Excellent interpersonal and communication skills.
• Basic understanding of clinical documentation and rehabilitation goals.
• Confidence to work autonomously in community settings, with appropriate clinical supervision.
• Full UK driving licence and access to a vehicle (community travel is essential).
Desirable
• Experience working within the NHS or private physiotherapy services.
• Knowledge of common neurological conditions (e.g. stroke, MS, Parkinson’s, brain injury).
• Familiarity with outcome measures and rehabilitation goal setting.
• Knowledge of safeguarding, risk assessments, and health & safety in community care.
• Basic first aid or manual handling training (training can be provided if not current).
